1. The charge for reconnecting cable service to a existing location or a transfer is $30.00, inside the city. Full hook ups are $50.00. Additional outlets added after the initial hook up is $30.00.

2. The monthly fee for basic service is $38.00 INCLUDING SALES TAX. Nursing Home discount rate is $22.00 per month.

3. The monthly fee for (1) connection on Home Box Office (HBO) is $10.95. For each additional connection, 95 cents is added to the rate up to four outlets. For four outlets and above $1.00 is added.

4. Cable fees are due the first day of each month, in advance, and delinquent after the tenth(10th) regardless of the hookup date. NO STATEMENTS ARE MAILED.

5. A late charge of $5.00 is added to your monthly rate if the fee is not received by the tenth (10th) of each month. There is also a $30.00 service charge and $5.00 mail fee on all returned checks.

6. Should you become delinquent on your monthly fees and we have to come to your home to collect , there is a service charge of $20.00.

7. When an account becomes one months past due, disconnection will be made immediately.

8. You can have your cable set up on bank draft, with your approval we will draw your monthly cable payment from your checking account.

9. If you wish to discontinue service or move, you must call in for a disconnection, or you will be charged until you do so.

10. If your cable is out, contact a neighbor to see if they are having trouble. If so, then call and report the cable trouble. Should the office be closed, you may leave a message. There is no charge for repairing cable, however, if the trouble is not the fault of the cable , but the fault of your set, vcr or illegal wiring, there will be a service charge of $20.00.
